The Humboldt County Recorder's Office is a passport acceptance facility. We will process your application and forward it to Passport Services. In order to apply for a passport book you will need the following:
· Evidence of Citizenship - (One of the following) 1. Certified Copy of Birth Certificate 2. Naturalization Certificate 3. Expired Passport · Two Identical Passport Photos (passport size specifically) · Personal Identification - i.e. Valid Driver's License · Fees: 16 & over (per person) 1. One check/money order payable to U.S. Department of State for $110.00* 2. One check/cash/money order payable to Humboldt County Recorder for $25.00 Under 16 (per person) 1. One check/money order payable to U.S. Department of State for $80.00* 2. One check/cash/money order payable to Humboldt County Recorder for $25.00 · Completed application that has NOT been signed. YOU MUST SIGN THE APPLICATION IN PERSON - WE MUST WITNESS YOUR SIGNATURE.
*For expedited service add $60.00 per application. CUSTOMER must provide an envelope with postage affixed for overnight delivery and pay an additional $14.96 fee for return overnight delivery to ensure receipt within 3 weeks.
